Week 27: The Brain Awakens
Baby is about the size of Cabbage (366mm head-to-heel)
The brain undergoes a growth spurt this week. The cerebral cortex develops more complex layers and the number of brain cells increases dramatically. The thalamus, which relays sensory information, becomes active. Your baby has regular sleep-wake cycles and may respond to your voice with movement. The lungs continue maturing but are not yet ready for independent breathing.
You may notice your shoes feeling tighter — foot swelling and even permanent size increases are common in pregnancy. The third trimester begins next week. Glucose tolerance test results may come back this week. Braxton Hicks may intensify.
What is important now
This is the final week of the second trimester. Review your birth plan and consider registering at your birthing facility. If GDM screening was positive, discuss a management plan with your provider.
Common symptoms
- decreased fetal movement — contact provider immediately
Wellness this week.
Nutrition
Brain development surge
- DHA omega-3 — critical during this brain growth spurt
- Choline (eggs, lean meat, cruciferous vegetables) for brain cell production
- Complex carbohydrates for steady glucose supply to the brain
- Adequate protein for continued growth
Exercise
Movement
Continue moderate activity; supportive footwear is increasingly important
Sleep
7-9 hours
Position: Left side with full body pillow support
· Frequent urination returns — keep a clear, lit path to the bathroom
· Limit fluids 2 hours before bed but ensure adequate daytime hydration
Mental wellness
Mindfulness
If diagnosed with GDM, know it's manageable and common (affects 6-9% of pregnancies). It doesn't mean you did anything wrong.
“Your baby's brain is blossoming — billions of neurons firing, learning, dreaming.”
Your timeline.
Week 6
21 weeks ago- routine
Checkpoint 1 — Mutterpass may be issued upon heartbeat confirmation
Week 19
8 weeks ago- routineWeeks 19–22
Checkpoint 2 — Screening Ultrasound
Choice between a basic biometric scan or detailed organ scan. Measures head circumference, abdominal circumference, femur length, and checks placental position.
GoalAssess fetal anatomy and growth, check for structural abnormalities, and verify placental location.
Week 28
Next week- routine
Checkpoint 1 — GDM screening
- routine
Checkpoint 2 — Rhogam if Rh-negative
Week 29
In 2 weeks- routineWeeks 29–32
Checkpoint 3 — Screening Ultrasound
Third and final routine ultrasound. Assesses fetal growth, position (cephalic/breech), amniotic fluid volume, and placental function.
GoalConfirm appropriate growth trajectory and baby's position for delivery planning.
Week 35
In 8 weeks- self pay igelWeeks 35–37
Checkpoint 1 — GBS Testing — Self-pay/IGeL
Rectovaginal swab to screen for Group B Streptococcus colonization. If positive, IV antibiotics are given during labor to prevent neonatal infection.
GoalIdentify GBS carriers to enable prophylactic treatment during delivery.
Safety: From this point forward, pay close attention to fetal movement patterns. If you notice a significant decrease, contact your provider for evaluation.
